Abstract
Objective:To explore the effects of different approaches of total hip arthroplasty on perioperative indicators and hip joint function in patients with femoral head necrosis.Method:The clinical data of 92 patients with femoral head necrosis admitted to the First Affiliated Hospital of Bengbu Medical University from June 2020 to June 2023 were retrospectively analyzed and divided into two groups according to surgical methods.44 patients receiving traditional posterolateral approach were used as control group and 48 patients receiving direct anterior approach were used as observation group.All patients were followed up for 6 months after surgery.Perioperative indexes,pain degree,hip function,life ability and complications were compared between the two groups.Result:The operation time of the observation group was longer than that of the control group,the incision length,getting out of bed activity and hospitalization time were shorter than that of the control group,and the intraoperative blood loss and postoperative drainage volume were less than that of the control group,and the differences were statistically significant(P<0.05).There was no significant difference in pain degree between the two groups before surgery and 6 months after surgery(P>0.05).Visual analogue scale(VAS)scores in the observation group were lower than those in the control group 24 h,72 h and 1 month after surgery,and the difference was statistically significant(P<0.05).There was no significant difference in preoperative hip function and life ability between the two groups(P>0.05).Six months after operation,the Harris score and modified Barthel index(MBI)in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).The complication rate of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Compared with the posterior lateral approach,the direct anterior approach has less trauma and postoperative bleeding during the initial total hip replacement surgery,which is beneficial for reducing pain,promoting the recovery of hip joint function and life ability,and has a lower risk of complications.关键词
