Abstract
Objective:To investigate the effect of dezocine plus dexmedetomidine on the incidence of urinary retention and sleep disorders in patients undergoing procedure for prolapse and hemorrhoids(PPH).Method:120 patients with mixed hemorrhoids admitted to the Second Hospital of Nanping City from January 2021 to January 2024 were selected as the study objects and randomly divided into control group and observation group,with 60 cases in each group.The control group was preanalgesic with dezocine,and the observation group was preanalgesic with dezocine combined with dexmedetomidine.The scores of Visual Analogue Scale(VAS)of 6 h,12 h,24 h,first defecation and first dressing change were compared between the two groups.The time of first urination,urinary retention occurred 7 days after operation,signs if urinary tract irritation and the incidence of catheteruzation were compared between the two groups.Pittsburgh Sleep Quality Index(PSQI)was used to evaluate the sleep quality of the two groups on day 1,day 3 and day 7 after surgery,and the incidence of sleep disorders on day 7 after surgery was analyzed.The social,psychological,physical and material functions of the two groups were evaluated by GQOLI-74 before and 7 days after surgery.The occurrence of adverse reactions(nausea,vomiting,itching,loss of appetite,anxiety)were recorded in the two groups.Result:The VAS scores of observation group at 6 h,12 h,24 h,first defecation and first dressing change were lower than those of control group,and the difference was statistically significant(P<0.05).The first urination time of the observation group was shorter than that of the control group,and the incidence of urinary retention,urinary tract irritation and urinary catheterization was lower than that of the control group,and the difference was statistically significant(P<0.05).The PSQI scores 1,3 and 7 days after surgery and the incidence of sleep disorders 7 days after surgery in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The quality of life scores of social,psychological,physical and material functions in the observation group were higher than those in the control group 7 days after surgery,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Preemptive analgesia with dizocine combined with dexmedetomidine can alleviate postoperative pain in PPH patients with mixed hemorrhoids,promote the recovery of bladder function,improve patients'sleep at night,reduce the incidence of urinary retention and sleep disorders,and improve patients'quality of life.关键词
