Abstract
Objective:To compare the clinical efficacy of bipolar trasurethral enucleation of the prostate(B-TUEP)and bipolar transurethral resection of the prostate(B-TURP)in the treatment of benign prostatic hyperplasia of 50-80mL.Method:A total of 115 patients with benign prostatic hyperplasia treated in the Second Hospital of Longyan,Fujian Province from January 2021 to December 2023 were selected as the study objects.According to different treatment methods,they were divided into enucleation group(B-TUEP treatment,60 cases)and electrical resection group(B-TURP treatment,55 cases).Perioperative data,urodynamics,prostatic status before and after operation and postoperative complications were compared between the two groups.Result:Surgery time,intraoperative blood loss,catheter indwelling time,bladder irrigation time and length of hospital stay in the B-TUEP group were shorter than those in the B-TURP group,and the mass of glandular tissues removed was higher than that in the B-TURP group,and the difference was statistically significant(P<0.05).Three and six months after surgery,the International Prostate Symptom Score(IPSS)and Quality of Life(QOL)scores decreased,and the difference was statistically significant(P<0.05).Postoperative IPSS scores of the B-TUEP group were lower than those of the B-TURP group,and the difference was statistically significant(P<0.05).But there was no significant difference in postoperative QOL scores(P>0.05).Three and six months after surgery,the maximum urine flow rate(Qmax)increased significantly and the residual urine volume(RUV)decreased significantly in both groups,and the difference was statistically significant(P<0.05).Three months after surgery,the improvements in Qmax and RUV were greater in the B-TUEP group than in the B-TURP group,and the difference was statistically significant(P<0.05).Six months after surgery,there was no statistically significant difference in Qmax or RUV between groups(P>0.05).The total incidence of complications in the B-TUEP group was 10.00%,and there was no statistically significant difference compared to the total incidence of 12.73%in the B-TURP group(P>0.05).Conclusion:For patients with benign prostatic hyperplasia of 50-80mL,B-TUEP and B-TURP both are safe and effective.However,the former can shorten the surgery time,reduce intraoperative blood loss,achieve faster postoperative recovery,and improve prostate condition and urodynamics more significantly.关键词
