基于电针的综合外治法对带状疱疹后遗神经痛患者的镇痛效应及生活质量的影响
Effects of a Comprehensive External Treatment Method Based on Electroacupuncture on Pain Relief and Quality of Life in Patients with Postherpetic Neuralgia

Abstract
Objective:To compare the pain-relieving effects and impacts on quality of life of a comprehensive external treatment method based on electroacupuncture in patients with postherpetic neuralgia(PHN).Methods:Ninety outpatients with PHN from the Acupuncture Department of Shaanxi Provincial Hospital of Chinese Medicine,treated from June 2017 to June 2021,were randomly divided into three groups:the electroacupuncture group,the electroacupuncture+tapping and cupping group,and the electroacupuncture+cotton moxibustion group,with 30 patients in each group.Each group received treatment for 10 days per course,with a total of 3 courses.Visual Analog Scale(VAS)scores and Dermatology Life Quality Index(DLQI)scores were recorded on days 1,30,and 90.Results:On days 30 and 90,VAS scores in all three groups decreased compared to before treatment(P<0.05),with the electroacupuncture+tapping and cupping group performing better than the electroacupuncture group and the electroacupuncture+cotton moxibustion group,and the electroacupuncture+cotton moxibustion group performing better than the electroacupuncture group(P<0.05).Among the10 categories of DLQI,scores in all three groups decreased on days30 and90 compared to before treatment(P<0.05).The electroacupuncture+tapping and cupping group and the electroacupuncture+cotton moxibustion group showed no statistically significant differences in scores for skin pain and itching,impact on housework,dressing,social interactions,work,relationship management,sexual life,and daily life between days 30 and 90(P>0.05).In contrast,scores for these 10 categories in the electroacupuncture group significantly rebounded on day 90 compared to day 30(P<0.05).On day 30,the electroacupuncture+tapping and cupping group had lower scores for skin pain and itching,emotional distress,impact on housework,dressing,and social interactions compared to the electroacupuncture group(P<0.05).There were no significant differences among the three treatment groups in scores for impact on exercise,work,relationship management,sexual life,and daily life on day 30(P>0.05).On day 90,the electroacupuncture+tapping and cupping group performed better than the electroacupuncture group and the electroacupuncture+cotton moxibustion group in these five categories(P<0.05).VAS scores were significantly positively correlated with DLQI scores.Conclusion:All three treatment methods have pain-relieving effects and can effectively improve patients'quality of life.The electroacupuncture+tapping and cupping group demonstrates more sustained pain relief and better quality of life improvements,particularly in terms of skin pain and itching,impact on housework,dressing,social interactions,work,relationship management,sexual life,and daily life.The electroacupuncture+cotton moxibustion group also has a notable long-term effect in reducing the impact on sexual life and daily life.关键词
