Abstract
Objective To conduct systematic review and meta analysis of the application value of physiological and opera-tive severity score for the enumeration of mortality and morbidity(POSSUM)system in predicting postoperative complications of spinal surgery,in order to evaluate the accuracy of POSSUM in predicting postoperative complications of spinal disease.Methods "Subject or title/abstract"combined with"spine"or"cervical spine"or"thoracic vertebra"or"lumbar spine"in the full text were searched by POSSUM through PubMed,Embase,the Cochrane Library,CNKI,Wanfang Data,WiP Chinese journals,and China Biomedical Literature Service System.The retrieval period was from the establishment of the database to July 2024.The number of people predicted by POSSUM scoring system and the number of actually observed complications were extracted.Relative risk(RR)was selected as the effect size to evaluate the prediction accuracy.Stata 16.0 was used for meta-analysis,publication bias and sensitivity analysis.Results A total of 144 relevant studies were re-trieved,and 6 studies were finally included,with a total of 1 001 patients.Among them,229 patients were predicted by POSSUM and 219 patients were actually complications.The RR value of POSSUM system for predicting the accuracy of complications was 1.03(95％CI:0.88-1.20,I2=2.2％,P=0.403),and the RR value of large sample subgroup was 1.12(95％CI:0.83-1.50,I2=44.9％,P=0.163),the RR value of the small sample subgroup was 0.97(95％CI:0.77-1.23,I2=0.0％,P=0.564),and the result of publication bias analysis using Egger method was P=0.348.The sensitivity analysis results showed that the included research samples were from the same population,without significant heterogeneity.Conclusion POSSUM system can accurately predict the incidence of postoperative complications in patients with spinal disease,and it is not affected by the sample size.However,the widespread use of this scoring system in spinal surgery requires further evaluation in larger,multi-center prospective studies.关键词
