Value of one-stop whole-brain dynamic volume CTA-CTP imaging technique for assessing collateral circulation in ischemic stroke

Abstract
Objective To investigate the value of one-stop whole-brain dynamic volume computed tomography angiography(CTA)-computed tomography perfusion(CTP)imaging for collateral circulation in ischemic stroke.Methods Eighty patients with ischemic stroke admitted to the Suzhou Kowloon Hospital,Shanghai Jiao Tong University School of Medicine from May 2023 to April 2024 were included in the study,including CTA,CTP,one-stop whole brain dynamic volume CTA-CTP imaging,and DSA,respectively.With DSA examination results as the gold standard,CTA,CTP and one-stop whole brain dynamic volume CTA-CTP imaging were analyzed,and the receiver operating characteristic(ROC)curves of CTA,CTP and one-stop whole brain dynamic volume CTA-CTP imaging were evaluated to measure the area under the ROC curve(AUC).Results In the 80 patients,the results of DSA examination showed that 24(30.00%)cases with poor collateral circulation,56(70.00%)cases with good collateral circulation;the results of the CTA examination showed that 27(33.75%)with poor collateral circulation,53(66.25%)cases with good collateral circulation;the results of CTP examination showed that 25(31.25%)cases of poor collateral circulation,55(68.75%)cases with good collateral circulation;the one-stop whole-brain dynamic volume CTA-CTP imaging examination results showed that 25(31.25%)cases of poor collateral circulation,55(68.75%)patients with good collateral circulation.Among the three imaging methods,the sensitivity(95.83%),the specificity(96.43%)and the accuracy(96.25%)of one-stop whole-brain dynamic volume CTA-CTP imaging examination was the highest,and the difference was statistically significant(P<0.05).The AUC values of the ROC curves of CTA,CTP,and one-stop whole brain dynamic volume CTA-CTP imaging examination were 0.821 3,0.818 7,and 0.910 8,respectively.Conclusion One-stop whole-brain dynamic volume CTA-CTP imaging can better evaluate the collateral circulation condition of ischemic stroke,and provide basis for clinical diagnosis and treatment and prognosis prediction.关键词
