血栓弹力图联合血清Hcy与NSE对缺血性脑卒中严重程度的诊断价值
Diagnostic value of thromboelastography combined with serum Hcy and NSE in the severity of ischemic stroke

Abstract
Objective To investigate the diagnostic value of thromboelastography(TEG)combined with serum homocysteine(Hcy)and neuron-specific enolase(NSE)for the severity of ischemic stroke(IS).Methods A total of 88 IS patients who were treated in the People's Hospital of Wuzhou from March 2018 to March 2023 were selected as the research subjects,and were divided into mild group(n=32),moderate group(n=29)and severe group(n=27)according to NIHSS score.In addition,42 cases of health examination in the Physical Examination Center were selected as control group.The TEG parameters(R,K,α,MA)of all subjects were detected by thromboelastograph.Enzyme-linked immunosorbent assay(ELISA)was used to detect the serum Hcy,and chemiluminescence method was used to detect the serum NSE level.Pearson correlation analysis was used to analyze the correlation between TEG parameters,serum Hcy and NSE levels and the severity of IS.Receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of TEG parameters(R,K,α,MA),serum Hcy and NSE levels for the severity of IS disease.Results The R,K in IS group were lower than those in control group(P<0.05),and the α,MA,and serum Hcy,NSE levels were higher than those in the control group(P<0.05).Compared with the mild/moderate group,the R,K in the severe group were decreased(P<0.001).Compared with the mild/moderate group,α,MA,and serum Hcy,NSE levels in the severe group were all increased(P<0.010).The R,K were negatively correlated with NIHSS score(r=-0.654,-0.632,P<0.001).The α,MA,and serum Hcy,NSE levels were positively correlated with NIHSS score(r=0.587,0.612,0.674,0.693,P<0.001).The ROC curve analysis showed that area under the curve(AUC)of the R,K,α,MA,serum Hcy and NSE were 0.857,0.882,0.897,0.828,0.871,0.890 and 0.996,respectively.The sensitivities were 88.89%,92.59%,77.78%,74.07%,88.89%,81.48%and 96.30%,respectively.The specificity was 73.77%,78.69%,91.80%,86.89%,80.33%,95.08%and 98.66%,respectively.Conclusion The diagnostic value of combined detection is higher than that of single diagnosis.TEG combined with serum Hcy and NSE has high diagnostic value for the severity of IS and has certain clinical application value.关键词
