IL-23/Th17免疫炎症轴在脑小血管病相关认知障碍中的临床意义
Clinical significance of IL-23/Th17 immune-inflammatory axis in the pathogenesis of cognitive impairment associated with cerebral small vessel disease

Abstract
Objective To analyze the clinical significance of the interleukin(IL)-23/helper T cell(Th)17 immune-inflammatory axis in the development of cognitive impairment(CI)associated with cerebral small vessel disease(CSVD).Methods A total of 134 CSVD patients admitted to the Neurology Department of Linfen People's Hospital from July 2019 to June 2022 were selected.These patients were divided into CSVD-CI group(Montreal cognitive assessment(MoCA)score<26,60 cases)and CSVD-NCI group(MoCA score≥26,74 cases).Additionally,50 healthy individuals who underwent physical examinations during the same period were included as control group.The levels of transforming growth factor β(TGF-β),IL-6,IL-17,and IL-23 within 24 hours of admission for all participants were measured.Logistic regression multivariate correlation analysis was used to obtain hazard ratios(HR),and the predictive efficacy of CI in CSVD patients was evaluated using ROC curves.Results Significant differences were observed between the CSVD-CI group and the CSVD-NCI group in age,smoking status,hypertension,diabetes,systolic blood pressure(SBP),diastolic blood pressure(DBP),fasting plasma glucose(FPG),history of stroke or transient ischemic attack(TIA),and other factors(P<0.05).The levels of TGF-β,IL-6,IL-17,and IL-23 in the CSVD-CI group were significantly higher than those in the CSVD-NCI group and the control group(P<0.01).Moreover,the levels in the CSVD-NCI group were also significantly higher than those in the control group(P<0.01).Logistic regression analysis showed that serum TGF-β,IL-6,IL-17,IL-23,diabetes,hypertension,and FPG were independent risk factors for CI in CSVD patients.ROC curve analysis demonstrated that the combined prediction of serum TGF-β,IL-6,IL-17,and IL-23 with the AUC of 0.955,and was higher than that of individual indicators.Conclusion Combined detection of serum TGF-β,IL-6,IL-17,and IL-23 can early predict CI in CSVD patients.Therefore,monitoring the trend of IL-23/Th17 immune-inflammatory axis-related factors in such patients may help early identification of those at high risk of cognitive impairment.关键词
