DTI imaging characteristics and risk assessment of aphasia improvement in patients with conduction aphasia after ischemic stroke

Abstract
Objective To investigate the imaging characteristics of diffusion tensor imaging(DTI)in patients with conduction aphasia after ischemic stroke and the influencing factors for aphasia improvement.Methods A total of 80 patients with conductive aphasia after ischemic stroke treated in Zhangjiakou First Hospital from January 2022 to January 2024 were selected,and the differences in DTI parameters on the left and right sides of the brain were analyzed,and the differences in clinical data of patients with effective and ineffective treatment were analyzed.Results The fractional anisotropy(FA)values of the Broca area,Wernicke area,and arcuate fiber bundle,apparent diffusion coefficient(ADC)and volume ratio anisotropy(VRA)values of the Broca area and arcuate fiber bundles on the left side of the affected brain were significantly lower than those on the right side(P<0.05),while the attenuation index(Exat)values were significantly higher than those on the right side(P<0.05).Age,the proportion of high school and below,the proportion of Boston aphasia severity scale(BDAE)grades 1-3,and NIHSS scores in patients with ineffective treatment were significantly higher than those of patients with effective treatment(P<0.05),while the Barthel index was significantly lower than those of patients with effective treatment(P<0.05).Logistic regression analysis showed that age,BDAE grading,and NIHSS score were the influencing factors for the efficacy of aphasia treatment(P<0.05).Conclusion There is a significant difference in DTI parameters between the left and right brain of patients with conduction aphasia after ischemic stroke,and the effectiveness of aphasia treatment is influenced by age,BDAE grading,and NIHSS score.关键词
