Abstract
Cognitive impairment has become an important public health problem in an aging society.In order to screen out high-risk groups for cognitive impairment in a timely manner and improve the health management and quality of life of people with cognitive impairment and their families,social organizations,medical institutions and community health service centers have taken the construction of medical consortium as an opportunity for medical social workers to take an active part in the hierarchical diagnosis and treatment of cognitive impairment and its management throughout the process.In the prevention and health care stage,community health education services;in the diagnosis and treatment stage,outpatient follow-up and short-term psychological counseling and support services;in the cognitive intervention stage,innovative group services using multiple elements such as tai chi,horticulture,life review,etc.;and in the late stage of the disease,case management services and family care support to alleviate the stress of caregivers.Improve the level of disease management and quality of life of families with cognitive impairment through holistic,graded and whole-process management,and explore effective ways to provide services for people with cognitive impairment embedded in medical associations.关键词
