Effect of transcranial direct current stimulation combined with upper limb robotic virtual scenario training on cognitive function in stroke patients

Abstract
Objective:To evaluate the effects of transcranial direct current stimulation(tDCS)combined with upper limb robotic virtual scenario training(ULR-VST)on cognitive function in stroke patients.Methods:150 stroke patients treated at the Affiliated Brain Hospital of Nanjing Medical University from June 2022 to June 2024 were enrolled.They were randomly divided into the conventional group(receiving standard cognitive rehabilitation),tDCS group(receiving tDCS alone)and combined group(receiving tDCS+ULR-VST),with 50 patients in each group.Cognitive function,behavioral performance,and activities of daily living(ADL)were compared across the three groups.Results:After 2 weeks of intervention,the combined group showed significantly higher scores on the mini-mental state examination(MMSE)and montreal cognitive assessment(MoCA)than the tDCS group and conventional group(P<0.05),with a rank order of combined group>tDCS group>conventional group.Similarly,the combined group outperformed the other two groups on the Behavioral assessment of dysexecutive syndrome(BADS)and Fugl-Meyer assessment for upper extremity(FMA-UE)(P<0.05).The Barthel index scores also followed the same trend:combined group>tDCS group>conventional group(P<0.05).Conclusion:Compared to conventional cognitive rehabilitation or tDCS alone,the combination of tDCS and ULR-VST shows better rehabilitation outcomes,which can effectively improve cognitive function,motor performance,and quality of life in stroke patients.关键词
