摘要

Abstract

Objective To investigate the effect of Shuangxi Gujiaobao(双膝骨胶宝,SXGJB)on fracture healing and the ex-pression of bone growth factor through TGF-β/Smad3 signaling pathway in rat fracture model.Methods The rats were randomly divided into control group,fracture model group,CaCO3D3 group,SXGJB low dose group(6.65 g/kg),middle dose group(13.3 g/kg)and high dose group(26.6 g/kg),24 in each group.The tibial fracture model was established in the fracture model group,CaCO3D3 group and each dose group of SXGJB and steel pin was used for intramedullary fixation.The fracture model group and the control group were given the same volume of normal saline,the CaCO3D3 group and the SXGJB low,middle and high groups were given the corresponding drugs.The samples were taken out at the 1st,3rd and 5th week of the experiment and HE staining was used to observe the pathological changes of bone tissue at the 1st,3rd and 5th week after fracture.The expression of cartilage and bone tissue in the fracture site was observed at the 1st,3rd and 5th week after fracture by staining with fuchsin.The levels of serum vascular endothelial growth factor(VEGF),procollagen type Ⅰ propeptide(PINP)and type Ⅰ collagen C-terminal peptide(CTX-1)were measured by Enzyme-Linked Immunosorbent Assay(ELISA)at the 5th week.The healing of tibia was observed by X-ray.The VEGF of tibia was observed by X-ray,the three-point bending method was used to measure the bio-mechanical strength of tibia 5 weeks after fracture,and Western Blot was used to detect the expression of transforming growth fac-tor-β(TGF-β)and Smad3 in tibia.Results Compared with those of the control group,the expressions of VEGF and PINP in the serum of the fracture model group increased(P<0.05,P<0.01).The expressions of TGF-β,VEGF,PINP and CTX-1 in SXGJB group was significantly increased(P<0.05,P<0.01),and the image score at the 3rd,4th and 5th week was higher than that in the model group(P<0.01).HE staining and fuchsin staining showed that the bone tissue of the fracture model group showed the following features in turn:part of the degenerative bone tissue and a large number of inflammatory cells infiltrated in the fracture area→the fracture gap was mainly composed of fibrous tissue→the fracture gap was filled with new cartilage tissue.And SXGJB can stimulate the formation of new cartilage,accelerate the process of fracture healing and promote bone maturity.Compared with those of the control group,the maximum load and bending stiffness of tibia in the fracture model group were signifi-cantly decreased(P<0.05,P<0.01).The maximum load and bending stiffness of tibia in SXGJB group were significantly higher than those in the model group(P<0.05,P<0.01).Western Blot showed that the expressions of TGF-β and SMAD3 protein in tibia of rats in SXGJB group were higher than those in the control group(P<0.05,P<0.01).Conclusion SXGJB can improve bone metabolism,promote fracture healing and improve bone quality,which may be related to the enhancement of VEGF expres-sion and TGF-β/Smad3 signaling pathway.
