Abstract
Objective A retrospective analysis was conducted on the laboratory detection process of a carbon diox-ide(CO₂)-dependent Escherichia coli strain isolated from the pleural effusion of a patient with aortic dissection,aiming to provide reference experience for the isolation,culture,and antimicrobial susceptibility testing of rare bacteria in clini-cal microbiology laboratories.Methods We extracted the pleural effusion from the patient with aortic dissection.The bacterium was cultured separately in a 5%CO₂ incubator and a standard incubator.Species identification was performed using the VITEK® MS microbial mass spectrometry system and the VITEK® Ⅱ compact microbial analysis system,and antimicrobial susceptibility testing was conducted using the Kirby-Bauer method.Results The strain grew well only in the 5%CO₂ incubator and did not grow in the standard incubator.Both mass spectrometry and the VITEK® Ⅱ compact GN card identified it as Escherichia coli.Antimicrobial susceptibility testing revealed that the strain produced extended-spectrum β-lactamases(ESBLs)and was resistant to ampicillin,cefazolin,cefuroxime,ceftriaxone,and cefotaxime.Conclusions This study identified an ESBL-producing CO₂-dependent Escherichia coli,highlighting the need for clini-cal microbiology laboratories to pay close attention to the isolation and identification of such rare strains.Proper culture conditions should be ensured to prevent missed detection,which could impact clinical diagnosis and treatment.关键词
