Abstract
Objective To analyze the diagnostic and preoperative staging value of dynamic contrast-en-hanced magnetic resonance imaging(DCE-MRI)combined with intravoxel incoherent motion imaging(IVIM)parameters in rectal cancer.Methods A prospective study was conducted on 167 patients with suspected rectal cancer admitted to our hospital from January 2021 to March 2024.All patients underwent DCE-MRI and IVIM examinations,followed by endoscopic biopsy for pathological confirmation.Pathologi-cal results served as the gold standard to evaluate the diagnostic performance of DCE-MRI combined with IVIM parameters for rectal cancer and its T/N staging.Results Pathological results confirmed 115 ma-lignant cases and 52 benign cases among the 167 suspected patients.Among the 115 malignant cases,T staging was distributed as T1(20 cases),T2(45 cases),T3(36 cases),and T4(14 cases).N staging was N0(72 cases),N1(27 cases),and N2(16 cases).DCE-MRI alone showed moderate agreement with patho-logical results for rectal cancer diagnosis(Kappa=0.503,P<0.001),while the combination of DCE-MRI and IVIM demonstrated substantial agreement(Kappa=0.805,P<0.001).The combined ap-proach significantly improved diagnostic accuracy,sensitivity,and specificity compared to DCE-MRI alone(P<0.05).For T/N staging,DCE-MRI alone showed moderate agreement with pathology(Kappa=0.507,0.559;P<0.001),whereas the combination of DCE-MRI and IVIM exhibited strong agreement(Kappa=0.803,0.808;P<0.001).Conclusion The combination of DCE-MRI and IVIM parameters enhances diagnostic accuracy for rectal cancer and improves preoperative staging assessment,demonstrating promising clinical utility.关键词
