Abstract
Objective To analyze the recurrence and metastasis and influencing factors in patients with low rectal cancer after laparoscopic transanal total mesorectal excision(TaTME),providing evidence for clinical practice.Methods A total of 202 patients with low rectal cancer admitted to our hospital from May 2020 to May 2022 were selected.According to the surgical methods,124 patients who underwent tra-ditional total mesorectal excision(TME)were assigned to the control group,and 78 patients who under-went TaTME were assigned to the observation group.According to the recurrence and metastasis of low rectal cancer patients in the observation group after 2 years,the patients were divided into a recurrence and metastasis group(17 cases)and a non-recurrence and metastasis group(61 cases).The surgical status,re-currence and metastasis of patients were analyzed,and the influencing factors of postoperative recurrence and metastasis of low rectal cancer patients with TaTME were analyzed by Logistic regression.Results The observation group had significantly shorter operative time(P＜0.05),less intraoperative blood loss(P＜0.05),and lower postoperative complication rates(P＜0.05)compared to the control group.A to-tal of 17 cases(21.79％)in the observation group experienced recurrence and metastasis,which showed no statistically significant difference compared to 32 cases(25.81％)in the control group(P＞0.05).Logis-tic regression analysis revealed that smoking,tumor differentiation grade,pathological type,and preopera-tive intestinal obstruction status were associated with recurrence and metastasis(P＜0.05).Conclusion Laparoscopic TaTME in the treatment of low rectal cancer is superior to TME in terms of operation time,intraoperative blood loss and postoperative complications.Smoking,tumor differentiation degree,pathological type and preoperative intestinal obstruction status are important factors affecting the metasta-sis and differentiation of patients with low rectal cancer with TaTME.Clinicians should pay more attention to these factors in postoperative management to improve the prognosis and quality of life of patients.关键词
