目的 探讨虚拟现实同步电刺激呼吸训练对慢性心力衰竭(CHF)患者运动耐力的影响.方法 选取2022年8月至2024年6月广西壮族自治区江滨医院住院或门诊的60例慢性心力衰竭患者,入组患者采用随机数字表法分为3组(A组、B组、C组),每组20例,所有入组对象均给予常规药物治疗,另外A组予常规呼吸训练、B组予电刺激呼吸训练(ER)、C组予虚拟现实同步电刺激呼吸训练(VR-ER).比较3组进行9周共36次训练前后心肺功能及运动耐力.结果 干预后3组峰值摄氧量(PeakVO2)、峰值功率(WR)、呼吸交换率(RER)、第1秒用力呼气容积占预计值的百分比(FE-V1%)、用力肺活量占预计值的百分比(FVC%)、第1秒用力呼气容积占用力肺活量的百分比(FEV1/FVC)、NT-proBNP、左室舒张末经(LVDD)、左室射血分数(LVEF)与治疗前比较以及3个组干预后的组间比较,差异均无统计学意义(P>0.05);干预后3组在呼吸峰值流速占预计值百分比(PEF%)的比较差异有统计学意义(P<0.05),VR-ER的改善效果最为显著,ER组次之;3组在PeakVO2、WR、RER、FEV1%、FVC%治疗前后差值的比较差异具有统计学意义(P<0.05),VR-ER的改善效果最为显著,ER组次之.结论 虚拟现实同步电刺激呼吸训练能改善CHF患者呼吸肌力量,同时在改善肺功能及运动耐力方面有积极影响.

Objective To investigate the effects of virtual reality synchronized electrical stimulation breathing training on exer-cise tolerance in patients with chronic heart failure(CHF).Methods A total of 60 CHF patients admitted to Jiangbin Hospital of Guangxi Zhuang Autonomous Region from August 2022 to June 2024 were selected and divided into three groups(A group,B group,and C group)by random number table method.Each group had 20 cases.All the enrolled subjects received conventional drug thera-py.In addition,A group received conventional breathing training,B group received electrical stimulation breathing training(ER),and C group received virtual reality synchronized electrical stimulation breathing training(VR-ER).The changes in cardiopulmonary function and exercise tolerance before and after 9 weeks of 36 training sessions in the three groups were compared.Results There were no significant differences in peak oxygen consumption(PeakVO2),peak power(WR),respiratory exchange ratio(RER),forced expiratory volume in one second of the predicted value(FEV1%),forced vital capacity of the predicted value(FVC%),forced expir-atory volume in one second of the predicted value(FEV1/FVC),NT-proBNP,left ventricular end-diastolic diameter(LVDD),and left ventricular ejection fraction(LVEF)before and after treatment in the three groups(P>0.05).There was a significant difference in the comparison of peak expiratory flow rate(PEF%)after intervention between the three groups(P<0.05),with VR-ER showing the most significant improvement,followed by ER group.There was a significant difference in the comparison of the changes in PeakVO2,WR,RER,FEV1%,and FVC%before and after treatment in the three groups(P<0.05),with VR-ER showing the most significant improvement,followed by ER group.Conclusion Virtual reality synchronized electrical stimulation breathing training can im-prove the breathing muscle strength of CHF patients and have a positive impact on improving pulmonary function and exercise tolerance.
