目的 探讨阴道扩张模型和阴道扩张联合卵巢切除模型两种盆底功能障碍(PFD)大鼠模型在模拟压力性尿失禁(SUI)和盆底器官脱垂(POP)症状及相关组织学改变方面的差异,并评估其作为研究工具的适用性.方法 选取34只Sprague Dawley大鼠,随机分为3组:空白对照组(NC组)7只、阴道扩张模型组(VD组)15只和阴道扩张联合卵巢切除模型组(VOX组)15只.VD组通过阴道扩张模拟产伤,VOX组在此基础上进行卵巢切除以模拟性激素减退.观察指标包括喷嚏实验、尿动力学检测、组织病理学染色和阴道单轴生物力学检测.检测时间点为阴道扩张术后第2周、4周和10周.结果 VD组和VOX组的喷嚏实验阳性率分别为60%和66.7%,均显著高于NC组(P<0.01),表明两种模型均成功诱导了SUI症状.VD组的腹压最大漏尿点压(ALPP)和最大膀胱容量(MBC)在术后2周显著降低,4周后逐渐恢复;VOX组的ALPP和MBC在术后2周、4周和10周均显著低于NC组(P<0.001),表明VOX模型的SUI症状持续时间更长.VD组和VOX组在术后2周均出现阴道和耻骨尾肌的组织结构损伤,但VD组在术后10周基本恢复,而VOX组的损伤更为严重且持续时间更长.VD组和VOX组的阴道组织最大拉伸距离显著增加(P<0.01),VOX组的最大拉伸强度显著低于NC组(P<0.05),表明VOX模型的组织力学性能改变更为显著.结论 阴道扩张模型适用于短期SUI研究,而阴道扩张联合卵巢切除模型更适合长期SUI和POP研究.两种模型在模拟PFD方面各有优缺点,研究者可根据具体需求选择合适的模型构建方法.

Objective To investigate the differences between two rat models of pelvic floor dysfunction(PFD)—the vaginal dis-tension(VD)model and the combined vaginal distension and ovariectomy(VOX)model—in simulating stress urinary incontinence(SUI)and pelvic organ prolapse(POP)symptoms,as well as associated histological changes,and to assess their applicability as re-search tools.Methods Thirty-four Sprague-Dawley rats were randomly divided into three groups:normal control group(NC,n=7),VD group(n=15),and VOX group(n=15).The VD group underwent vaginal distension to simulate childbirth injury,while the VOX group additionally underwent ovariectomy to simulate sex hormone deficiency.Evaluations included sneeze tests,urodynamic studies,histopathological staining at 2 weeks,4 weeks,and 10 weeks post-surgery and vaginal uniaxial biomechanical testing.Results The positive rates of sneeze test in the VD group and the VOX group were 60%and 66.7%,respectively,both significantly higher than the NC group(P<0.01),indicating successful induction of SUI symptoms.The abdominal leak point pressure(ALPP)and maximum bladder capacity(MBC)in the VD group significantly decreased at 2 weeks post-surgery but gradually recovered by 4 weeks.In contrast,the VOX group showed significantly lower ALPP and MBC at 2 weeks,4 weeks,and 10 weeks compared to the NC group(P<0.001),indicating more prolonged SUI symptoms.Both VD and VOX groups exhibited histological damage in the vagina and pubococcygeus muscle at 2 weeks,but the VD group largely recovered by 10 weeks,whereas the VOX group showed more severe and persistent damage.The maximum elongation distance of vagina significantly increased in both VD and VOX groups(P<0.01),while the VOX group showed significantly reduced maximum tensile strength compared to the NC group(P<0.05),reflecting pro-nounced biomechanical property changes.Conclusion The VD model is suitable for short-term SUI studies,while the VOX model better mimics long-term SUI and POP progression.Both models have distinct advantages and limitations in simulating PFD;research-ers should select the appropriate model based on specific objectives.
