代谢综合征合并脑小血管病相关睡眠障碍的研究进展OA

Research progress on sleep disorders related to metabolic syndrome combined with cerebrovascular disease

代谢综合征(MS)、脑小血管病(CSVD)及睡眠障碍三者常合并出现,此前研究多局限于单个疾病方向,随着研究的深入,发现MS合并CSVD患者睡眠障碍的发病率显著增加,而睡眠障碍患者的MS和CSVD风险亦明显增加,三者关系密切且发病机制存在部分重叠.本文综述了 MS、CSVD以及睡眠障碍发病的共同机制,分析了睡眠障碍的成因,发现MS与CSVD的共同机制涉及胰岛素抵抗、脂肪酸代谢障碍、氧化应激与慢性炎症;MS与睡眠障碍共同机制涉及胰岛素抵抗、肥胖、氧化应激及代谢紊乱;CSVD与睡眠障碍的循环机制涉及神经功能紊乱、慢性脑缺血和低灌注.MS影响睡眠障碍可分为呼吸性问题与非呼吸性问题,其中呼吸性问题主要为阻塞性睡眠呼吸暂停低通气综合征;CSVD影响睡眠障碍主要与脑低灌注致脑组织受损及体液调节因素相关.三者互为危险因素,可加速各自的发展进程,最终形成恶性循环,早期评估与及时干预有助于打破这一恶性循环.

The incidence of metabolic syndrome(MS),cerebrovascular disease(CSVD)and sleep disorder,often occur together,Previous studies were mostly limited to the direction of a single disease.With the deepening of the study,it was found that the inci-dence rate of sleep disorders in MS patients with CSVD was significantly increased,and the risks of MS and CSVD in patients with sleep disorders were also significantly increased.The three are closely related and the pathogenesis is partially overlapping.This arti-cle summarizes the common mechanisms of MS,CSVD,and sleep disorders,and analyzes the causes of sleep disorders.The common mechanism between MS and CSVD involves insulin resistance,fatty acid metabolism disorders,oxidative stress,and chronic inflam-mation.The common mechanisms of MS and sleep disorders involve insulin resistance,obesity,oxidative stress,and metabolic disor-ders.The circulatory mechanism of CSVD and sleep disorders involves neurological dysfunction,chronic cerebral ischemia,and hy-poperfusion.The impact of MS on sleep disorders can be divided into respiratory and non respiratory problems,with respiratory prob-lems mainly being obstructive sleep apnea hypopnea syndrome.The impact of CSVD on sleep disorders is mainly related to brain tis-sue damage caused by cerebral hypoperfusion and fluid regulation factors.Through comprehensive analysis,the above three factors are each other's risk factors,which can accelerate their respective development processes and ultimately form a vicious cycle.Early as-sessment and timely intervention can help break this vicious cycle.
