骨科驻科药师基于加速康复外科理念的全程化药学服务实践与探讨
Practice and Discussion of Whole-Process Pharmaceutical Care in Orthopedic Department Based on the Concept of Enhanced Recovery After Surgery

Abstract
OBJECTIVE:To explore the practice of whole-process pharmaceutical care provided by resident pharmacists in Orthopedic Department based on the concept of enhanced recovery after surgery.METHODS:The whole-process pharmaceutical care provided by resident pharmacists in Orthopedic Department was introduced around pharmaceutical evaluation,medication reconciliation,management of perioperative prophylactic administration,medical order review of therapeutic drugs,postoperative pharmaceutical care,and individualized medication guidance.By analyzing the indicators of the pharmaceutical care capabilities of clinical pharmacists and important assessment indicators of the hospital in the first quarter of 2023(before residency)and the first quarter of 2024(after residency),the effect of pharmaceutical care provided by clinical pharmacists after residency was evaluated.RESULTS:After the residency of clinical pharmacists,the evaluation indicators of pharmaceutical care capabilities were significantly improved compared to before residency;the etiological submission rate before antibiotics treatment increased by 100.03%(from 33.33%to 66.67%),the intensity of antibiotics decreased by 65.21%[from 36.91 DDDs/(100 人·d)to 12.84 DDDs/(100 人·d)],the utilization rate of antibiotics decreased by 12.88%(from 51.30%to 44.69%),the prophylactic administration rate of antibiotics in type Ⅰ incision surgery decreased by 9.09%(from 55.00%to 50.00%),the standardized prevention rate in high-risk patients with venous thromboembolism increased by 3.05%(from 93.16%to 96.00%),and the rate of standardized nutritional treatment for patients with nutritional risks increased by 72.41%(from 58.00%to 100.00%).CONCLUSIONS:Based on the concept of enhanced recovery after surgery,resident pharmacists carry out whole-process pharmaceutical care in the Orthopedics Department,which can improve the capabilities and effects of pharmaceutical care and make pharmaceutical care closer to the clinic and patients.关键词
