目的 探讨在极早早产儿及极低出生体重儿早期呼吸管理期间应用经鼻间歇正压通气(NIPPV)的效果.方法 回顾性分析2021 年6 月至2023 年1 月博尔塔拉蒙古自治州人民医院收治的 100 例极早早产儿或极低出生体重儿的基本资料,其中采取经鼻持续气道正压通气(NCPAP)联合早期呼吸管理的 52 例患儿为对照组,实施早期呼吸管理联合NIPPV的48 例患儿为观察组.比较 2 组的治疗相关时间、辅助通气相关指标和血气分析指标.结果 观察组无创通气、用氧及住院治疗的时间均短于对照组,机械通气率和肺表面活性物质使用率均低于对照组,呼吸暂停次数少于对照组,呼吸暂停程度轻于对照组(P均<0.05).治疗12 h、24 h和出院时,2 组患儿的血气分析指标(氧合指数和动脉血氧分压)参数均较治疗前改善(P均<0.05),且观察组均优于同期对照组(P均<0.05).结论 在极早早产儿及极低出生体重儿早期呼吸管理期间行NIPPV能改善辅助通气相关指标和血气分析指标,且有助于缩短治疗相关时间.

Objective To investigate the effect of nasal intermittent positive pressure ventilation(NIPPV)in the early respiratory management of very preterm infants and very low birth weight infants.Methods A retrospective analysis was conducted on the basic information of 100 very preterm infants or very low birth weight infants admitted to Bortala Mongolian Autonomous Prefecture People's Hospital from June 2021 to January 2023.Among them,52 patients who received nasal continuous positive airway pressure(NCPAP)combined with early respiratory management were enrolled as the control group,and 48 patients who received early respiratory management combined with NIPPV were enrolled as the observation group.The treatment-related time,assisted ventilation-related indicators,and blood gas analysis indicators between the two groups were compared.Results The observation group exhibited shorter non-invasive ventilation,oxygen use,and hospitalization time compared to the control group.The mechanical ventilation rate and pulmonary surfactant utilization rate were lower than those of the control group.The times of apnea were less than those of the control group,and the degree of apnea was milder than that of the control group(P<0.05).At 12 h and 24 h of treatment and discharge,the blood gas analysis indicators(oxygenation index and arterial partial pressure of oxygen)of both groups of children improved compared to before treatment(P<0.05),and the observation group was superior to the control group during the same period(P<0.05).Conclusion NIPPV in the early respiratory management of very preterm infants and very low birth weight infants can improve assisted ventilation-related indicators and blood gas analysis indicators,and help shorten treatment-related time.
