重度脑小血管病负荷患者排尿功能障碍与自主神经功能相关性研究
Association between urinary dysfunction and autonomic nervous system function in patients with severe cerebral small vessel disease burden

Abstract
Objective To investigate the correlation between urinary dysfunction and autonomic nervous function in patients with severe disease burden of cerebral small vessel disease(CSVD).Methods A retrospective analysis was conducted on 50 patients with severe disease burden of CSVD admitted to the Department of Neurology,Lishui People's Hospital from January 2021 to October 2023.According to the Overactive Bladder Syndrome Score(OABSS),patients were divided into an overactive bladder(OAB)group and a non-OAB group,with 25 cases in each group.Another 25 healthy individuals underwent health examination in the same hospital during the same period were selected as normal controls.The urinary function of participants was evaluated using OABSS,and autonomic nervous function was assessed using heart rate variability(HRV).The HRV parameters of participants in the three groups were compared and analyzed,followed by an exploration of the correlation between urinary dysfunction and HRV parameters using partial correlation analysis.Results Statistically significant differences were observed among the three groups in age,body mass index,Montreal Cognitive Assessment(MoCA)scores,Urgent Urination Score,Frequency Urination Score,Urinary Incontinence Score,Nocturia Score,and total OABSS Score(all P<0.05).Significant differences were also found in standard deviation of RR intervals index(SDNNindex),root mean square differences of successive RR intervals(rMSSD),percentage of RR intervals with>50 ms of variation(PNN50),high-frequency power,and low-frequency power(all P<0.05).Compared to the normal control group,the OAB group exhibited significantly lower SDNNindex,rMSSD,PNN50,high-frequency power,and low-frequency power,while the non-OAB group showed reduced rMSSD,PNN50 and high-frequency power(all P<0.05).All HRV parameters except low-frequency power demonstrated negative correlations with total OABSS scores,while SDNNindex,rMSSD and the triangular index were negatively correlated with Urgent Urination and Nocturia Score;PNN50 and high-frequency power were negatively associated with Urgent Urination Score;and very low-frequency power showed a negative correlation with Nocturia Score(all P<0.05).Conclusion The urinary dysfunction in patients with severe disease burden of CSVD is associated with autonomic nervous dysfunction.Targeting autonomic dysfunction is expected to be a potential intervention target for the treatment of urinary dysfunction of CSVD patients.关键词
