Abstract
Objective To analyze the polymorphism of heat shock protein 70(HSP70)gene,the expression characteristics of serum micro RNA-21(miRNA-21),and the prognostic influencing factors in elderly patients with chronic heart failure(CHF).Methods A total of 106 elderly patients with CHF admitted to Wenzhou Hospital of Integrated Traditional Chinese and Western Medicine from January 2020 to January 2022 were prospectively selected as the observation group,and another 60 healthy individuals who underwent physical examinations in the same hospital during the same period were selected as the control group.Peripheral venous blood of patients in the observation group was collected on the day after admission,while that of participants in the control group was collected on the day of physical examination.Polymerase chain reaction(PCR)technology was used to detect and analyze the polymorphisms of HSP70-1 gene+190G/C,HSP70-2 gene+1267A/C,and HSP70-hom gene+2437T/C.The level of serum miRNA-21 was also measured.The patients were followed up for one year.According to whether major adverse cardiovascular events occurred,they were divided into a poor prognosis subgroup of 17 cases and a good prognosis subgroup of 89 cases.Multivariate logistic regression analysis was used to analyze the factors influencing elderly CHF.Results There were no statistically significant differences between the two subgroups in the genotypes and alleles of HSP70-1 gene+190G/C,and HSP70-hom gene+2437T/C(all P>0.05).The proportions of AA and allele A in the HSP70-2 gene+1267A/C genotype in the observation group were less than those of the control group,and the expression level of serum miRNA-21 was also lower,with differences statistically significant(all P<0.05).Compared with the good prognosis subgroup,the poor prognosis subgroup had an older age,a larger proportion of NYHA cardiac function grade Ⅳ,a lower proportion of HSP70-2 gene+1267A/C genotype AA,and a lower expression level of serum miRNA-21;the differences were statistically significant(all P<0.05).Multivariate logistic regression analysis showed that age,NYHA cardiac function classification,HSP70-2 gene+1267A/C genotype,and serum miRNA-21 were all factors affecting prognosis(all P<0.05).Conclusion There are abnormalities in both HSP70 gene polymorphism and serum miRNA-21 in elderly patients with CHF.Age,NYHA cardiac function classification,HSP70-2 gene+1267A/C genotype and serum miRNA-21 are the influencing factors for the prognosis of elderly patients with CHF.关键词
