双心疾病的中医药治疗研究思考及理论范式探析
Research on Theoretical Paradigm of Chinese Medicine Treatment for Psycho-cardiological Disease

Abstract
Traditional Chinese medicine has gained a lot from the research on the rationale,prescription,and micro-mechanism of psy-cho-cardiological disease,and has gradually formed a treatment method based on dredging the Liver and regulating the Qi,supplemen-ted by promoting blood stasis,resolving Phlegm and tranquilizing the mind.However,the treatment of psycho-cardiological disease by Chinese medicine is still faced with many problems and deficiencies,which are mainly reflected in the following aspects:(1)Limita-tions in the research of etiology and pathogenesis:Current research focuses on"Qi stagnation""blood stasis""Phlegm and Dampness""Fire and Heat""Heat",and so on,yet the side of"Qi deficiency""Yang deficiency"and other Zheng Qi deficiency,which also plays an important role in the pathogenesis of psycho-cardiological disease is ignored.There is a bias in the understanding of"Heart is the master of blood and meridians"and"Heart is the master of Spirit",and there is a lack of modern medical knowledge of the deeper in-terpretation,which leads to the uncomplete theoretical system of Chinese medicine treatment of psycho-cardiological disease.(2)In-sufficient combination of identification of syndrome and identification of disease:Chinese medicine emphasizes the identification of syn-drome,but the clinical symptoms of psycho-cardiological disease are complex and varied,so if we rely too much on the identification of syndrome and neglect the identification of disease,the clinical efficacy may be reduced.Therefore,how to combine the identification of diseases based on accurate identification of syndrome and identification of diseases is the key to the future treatment of psycho-cardio-logical disease in Chinese medicine.(3)The singularity of the research method of syndromes:the current research method is still domi-nated by traditional ideas and there is a lack of the combination with modern scientific and technological means,which limits the depth and breadth of the research.(4)Insufficient innovation of basic research:the existing research is highly repetitive,without unique theo-retical guidance,which is not very innovative,and has limited clinical guidance value.Therefore,under the guidance of the overall idea of clinical diagnosis and treatment of psycho-cardiological diseases,which is"to define the disease with syndrome and combine the syndrome with the disease",a comprehensive treatment strategy of"four-step approach"can be adopted.First of all,the"six-merid-ian system in syndrome differentiation"in Typhoid Theory should be used as the outline,namely,starting from Yin-Yang,Biao-Li,Cold-Heat and Deficiency-Excess as well as Taiyang-Shaoyin Biao syndrome and Yangming-Taiyin Li syndrome and the semi-Biao and semi-Li syndrome of Shaoyang-Jueyin syndrome,and the syncope,so as to define the syndrome of the psycho-cardiological disease with the most basic syndrome identification of the six-meridian system.Secondly,"San Jiao should be considered as the fo-cus".Psycho-cardiological disease is mostly in the semi-Biao and semi-Li,which includes the Fu of San Jiao,and it should be con-sidered based on San Jiao syndrome identification.In this way the nature of the disease can be further identified to exactly located the disease.Then,the detailed identification of disease mechanism should be done by"taking the Zang and Fu into account"in framework of the six-meridian system the changes of the disease mechanism.Finally,do not forget the"blood stasis""Phlegm,Water and Damp-ness""Fire and Heat""stagnation of Qi"as well as"deficiency"and other pathologic factors.关键词
