零平面在鉴别中枢和外周病变导致的持续水平性背地性眼震中的意义
Significance of null point in distinguishing persistent horizontal apogeotropic nystagmus caused by central and peripheral disorders

Abstract
Objective To investigate the significance of null point in the persistent horizontal apogeotropic nystagmus caused by central and peripheral disorders.Methods Sixteen patients with vertigo in Tongzhou Hospital of Dongzhimen Hospital from January 1,2021 to January 31,2024 were retrospectively included.There were 9 cases of central disorders and 7 cases of peripheral disorders(cupulolithiasis/heavy cupula diseases).Results Patients in the two groups were evaluated in terms of general data(gender,age,treatment mode,onset to treatment time,smoking history),previous medical history(hypertension,diabetes,hyperlipidemia,carotid atherosclerosis and cerebral infarction),clinical symptoms and signs(visual rotation,nausea and vomiting,headache,hypoesthesia,gait instability,positive ataxia test),and nystagmus characteristics(spontaneous nystagmus and nystagmus angular velocity).There was no significant difference(P＞0.05).Gaze-evoked nystagmus and positional nystagmus induced by multiple planes were not present in all patients with central disorders.Null point was only seen in patients with peripheral disorders(cupulolithiasis/heavy cupula diseases).Conclusion The presence of null point can be used as one of the basis for distinguishing central and peripheral disorders of persistent horizontal apogeotropic nystagmus.关键词
