早期僵直型帕金森病脑白质微观结构的变化研究
Microstructure changes of white matter in early akinetic-rigid Parkinson's disease

Abstract
Objective To investigate the microstructural abnormalities in white matter tracts of early akinetic-rigid Parkinson's disease(AR-PD)patients using diffusion tensor imaging(DTI),aiming to reveal specific patterns of white matter damage in AR-PD.Methods A total of 51 AR-PD patients and 61 healthy controls were included in the study.Tract-based spatial statistics(TBSS)were applied to analyze DTI data.Fractional anisotropy(FA)values were compared between the two groups to assess the changes in white matter microstructure.Results Compared to the healthy controls,early AR-PD patients showed significantly reduced FA values in the bilateral anterior thalamic radiation,corticospinal tracts,superior longitudinal fasciculus,inferior longitudinal fasciculus,and the genu and body of the corpus callosum.These changes were strongly correlated with the Movement Disorder Society-sponsored unified Parkinson's disease rating scale(MDS-UPDRS Ⅲ),Hoehn-Yahr(H-Y)stage,and geriatric depression scale(GDS)scores(P<0.05).Conclusion Early AR-PD patients exhibit significant microstructural changes in white matter tracts,particularly reduced FA values in the bilateral anterior thalamic radiation,corticospinal tracts,and several other brain regions.These changes are closely associated with motor dysfunction and may also influence disease progression.White matter microstructural changes could serve as potential imaging biomarkers for the early diagnosis of AR-PD,providing scientific evidence for early intervention and treatment.关键词
