伴海马硬化性颞叶癫痫患者皮层及皮层下形态学特征的变化及评价
Changes and evaluation of cortical and subcortical morphological features in temporal lobe epilepsy patients with hippocampal sclerosis

Abstract
Objective To measure the cortical and subcortical gray matter volume,cortical thickness and cortical surface area in temporal lobe epilepsy with hippocampal sclerosis(TLE-HS)patients,and to analyze the clinical significance of the differences.Methods A total of 101 TLE-HS patients(TLE-HS group)and 220 healthy volunteers(HC group)were enrolled.All subjects underwent MRI examination to obtain high-resolution 3D-T1 MPRAGE structural image data,which were segmented and quantified by Freesurfer software based on Aseg and DKT maps.The morphological parameters were compared between the two groups using the general linear model(GLM)with age,sex,education level and total intracranial volume as covariates.Spearman correlation analysis was used to analyze the correlation between the subcortical nucleus volume with difference and disease duration.Results The volumes of the left pallidum((1 959.48±248.76)mm3)and the right thalamus((7 398.25±1 043.41)mm3)in the TLE-HS group were significantly reduced,while the volumes of the right hippocampus((4 131.69±785.72)mm3),amygdala((1 858.13±360.08)mm3)and the cortical gray matter((237 967.87±36 128.67)mm3)in the right hemisphere were significantly increased(P<0.05).The abnormal cortical thickness and cortical surface area in the TLE-HS group were mainly manifested in the temporal lobe and frontal lobe.Conclusion The morphological changes of cerebral cortex and subcortical structures in TLE-HS patients provide important imaging evidence for further understanding the pathophysiological mechanism of TLE-HS.关键词
