横纹肌样脑膜瘤的MRI表现及病理基础
MRI findings and pathological basis of rhabdoid meningioma
摘要
Abstract
Objective To investigate the MRI and pathological features of rhabdoid meningioma(RM).Methods MRI and pathological data of 15 patients with RM confirmed by surgery and pathology were retrospectively analyzed to investigate the MRI manifestations.Results Among the 15 cases of RM.9 cases were solid and 6 cases were solid-cystic.The tumors were located in the convex surface of the brain in 4 cases,next to the sagittal sinus in 3 cases,the convex surface of the brain and next to the sagittal sinus in 2 cases,the brain parenchyma in 2 cases,next to the sella in 1 case,the lateral ventricle in 1 case,the pontine cerebellar angle area in 2 cases.Among the 15 cases of RM,10 cases were irregular and 10 cases had unclear local boundaries.There were 4 cases of intratumoral cystic change or necrosis,2 cases of peritumoral cystic change,and 3 cases of hemorrhage.The solid part of the tumor was mainly isointensity or slightly hypointensity on T1WI,and mainly isointensity or slightly hyperintensity on T2WI,and 1 case was hypointensity.The necrotic and cystic parts showed hypointensity on T1WI and hyperintensity on T2WI.The solid part of the tumor was mainly hyperintensity and slightly hyperintensity on DWI,and mainly hypointensity or slightly hypointensity on ADC.Enhanced scanning showed significant uneven enhancement in 9 cases.There were severe edema in 4 cases,moderate edema in 3 cases,and mild edema in 1 case.There were adjacent skull destruction in 3 cases,and transcranial growth in 1 case.Five cases showed short and thick dural tail sign.Pathology showed multifocal rhabdomyoma cells and a variety of classical meningioma morphological structures,often accompanied by necrosis,cystic changes,and hemorrhage.The immunohistochemistry of RM showed the positive expression of EMA and Vim,with a mean Ki-67％ proliferation index of 24％.Conclusion MRI findings of RM have certain characteristics,which are solid or solid-cystic,prone to cystosis,necrosis,and hemorrhage,and may also be accompanied by bone destruction.The final diagnosis still depends on pathological examination.关键词
