尿白蛋白排泄率、尿白蛋白/肌酐比值对糖尿病肾脏疾病进展的预测价值及其影响因素分析
Predictive value and influencing factors of urinary albumin excretion rate and urinary albu-min-to-creatinine ratio in the progression of diabetic kidney disease

Abstract
Aim To investigate the predictive value of urinary albumin excretion rate(UAER)and urinary albumin-to-cre-atinine ratio(UACR)in the progression of diabetic kidney disease(DKD)and analyze their influencing factors.Methods A to-tal of 120 DKD patients were enrolled as the DKD group,and 100 healthy subjects were selected as the control group.UAER and UACR were measured by immunoturbidimetry.The DKD group was further divided into mild-to-moderate(G3-4,n=82)and severe(G5,n=38)subgroups.Spearman correlation,multivariate Logistic regression,and ROC curve analysis were performed to evaluate the associations and diagnostic efficacy.Results UAER and UACR were significantly higher in the DKD group than those in the control group(P<0.05).The severe subgroup had longer disease duration,higher UAER and UACR,and lower eGFR than the mild-to-moderate subgroup(P<0.05).UAER and UACR were negatively correlated with eGFR(r=-0.513,-0.428,P<0.05).Logistic regression showed that disease duration,UAER,and UACR were risk factors,while eGFR was a protective factor(P<0.05).The combination of UAER and UACR achieved an AUC of 0.849,with 85.67%sensitivity and 78.43%specificity,outperforming single indicators(P<0.05).Conclusion UAER and UACR are elevated in DKD patients and can be used for disease assess-ment.Their combination improves the prediction of severe DKD.Disease duration,UAER,and UACR are risk factors,while eGFR is a protective factor.关键词
