CT features of lung adenocarcinoma with different pathological subtypes and their correlation with serum tumor marker levels and EGFR gene mutations

Abstract
Aim To investigate the CT features of different pathological subtypes of lung adenocarcinoma and their correlation with serum tumor markers carcinoembryonic antigen(CEA),carbohydrate antigen 125(CA125)levels,and epidermal growth factor receptor(EGFR)gene mutations.Methods 80 patients with lung adenocarcinoma in our hospital were divided into pre-invasive group(19 cases),microinvasive group(22 cases),and invasive group(39 cases)based on their surgical and pathologi-cal results.All patients underwent CT scanning before surgery to analyze the CT features of different pathological subtypes of lung ad-enocarcinoma.The expression levels of serum tumor markers CEA and CA125 in lung adenocarcinoma under different CT signs,as well as CT features of patients carrying EGFR gene mutation were compared.Results The proportion of lesion lung interface blur,lobulation sign,spiculation sign,vacuolar sign,and nodule diameter in patients with lung adenocarcinoma followed an order of the invasive group>microinvasive group>pre-invasive group(P<0.05);The proportion of pure ground glass nodules was in the order of invasive group<microinvasive group<pre-invasive group(P<0.05);The proportion of pleural indentation sign and air bronchogram sign in the invasive group were higher than those in the microinvasive group and pre-invasive group(P<0.05).The serum tumor markers CEA and CA125 levels were higher in patients with CT signs of lobulation,vacuolar sign,and nodule diameter ≥ 20 mm com-pared with the patients without lobulation sign,vacuolar sign,and nodule diameter<20 mm(P<0.05).Out of 80 patients with lung adenocarcinoma,60 cases(75.00%)developed EGFR gene mutations,with 19Del and L858R mutations being the main types of mu-tations.The proportion of pure ground glass nodules in EGFR mutant patients was higher than that in EGFR wild-type patients(P<0.05).Conclusion CT signs can reflect the invasion degree of lung adenocarcinoma to a certain extent,and its imaging manifes-tations are closely related with serum tumor marker CEA and CA125 expression levels,with a higher EGFR gene mutation rate among patients with pure ground-glass nodules.关键词
