大分割调强放疗结合EP化疗方案对局限期小细胞肺癌患者血清肿瘤标志物、免疫功能的影响及其不良反应观察
Observation of the effects and adverse reactions of large fractionated intensity-modulated ra-diotherapy combined with EP chemotherapy on serum tumor markers and immune function in patients with locally limited small cell lung cancer

Abstract
Aim To observe the effects and adverse reactions of large fractionated intensity-modulated radiotherapy combined with etoposide plus cisplatin(EP)chemotherapy on serum tumor markers and immune function in patients with locally limited small cell lung cancer.Methods Totally 158 patients with locally limited small cell lung cancer in our hospital were divided into a con-trol group(EP regimen+hyperfractionation radiotherapy regimen)and an observation group(EP regimen+large segment intensity-mod-ulated radiotherapy).The therapeutic efficacy and incidence of adverse reactions in two groups were observed.The levels of im-mune function indicators[CD4+,CD3+,CD8+,CD4+/CD8+],serum tumor markers[cyto-keratin 19 fragment antigen 21-1(CY-FRA21-1),carcinoembryonic antigen(CEA),pro-gastrin-releasing peptide(ProGRP),carbohydrate antigen 125(CA125)],and serum metastasis and invasion related indicators[matrix metalloproteinase(MMP)-9,MMP-2,cell proliferation nuclear antigen-67(Ki-67),vascular endothelial growth factor(VEGF)]before and after treatment between two groups were compared.Results The total effective rate of the observation group was higher than that of the control group,and the incidence of adverse reactions was lower than that of the control group(P<0.05).After treatment,the levels of CD4+,CD3+,and CD4+/CD8+in peripheral blood of both groups were increased,and the observation group was higher than the control group(P<0.05);The levels of CD8+,CYFRA21-1,CEA,CA125,ProGRP,MMP-2,Ki-67,MMP-9,and VEGF were all decreased in both groups,and the observation group was lower than the control group(P<0.05).Conclusion The combination of large segment intensity-modulated radiotherapy and EP chemotherapy regimen can effectively reduce the levels of tumor markers in patients with limited stage small cell lung cancer,control the levels of metastasis and invasion related indicators,regulate their immune function,and have significant and safe therapeutic effects.关键词
