Meta-synthesis of Qualitative Studies on Patients'Experiences with Implantable Cardioverter Defi-brillator(ICD)Deactivation

摘要
Abstract
Objective To systematically integrate the current status and barriers in decision-making regarding ICD deactivation at the end of life,aiming to optimize decision support strategies and enhance comfort and nursing quality for end-of-life patients.Methods Qualitative studies on patients'preferences and experiences with ICD deactivation at the end of life were searched online in CNKI,CBMdisc,Wanfang Data,Web of Science,PubMed and Embase from their establishment to January 10,2024.The quality of the included articles was evaluated according to 2016 qualitative research quality evaluation criteria of the Joanna Briggs Institute(JBI)Centre for Evidence-based Healthcare in Australia.Results A total of 11 articles were included,from which 31 complete research topics were extracted and organized into 4 categories and 3 synthesized results.Synthesized results:① Patient decision-making characteristics and physician-patient collaboration needs:Decision-making preferences driven by deteriorating quality of life and electrical shock trauma,and decision-making timing choices oriented toward preserving autonomy;② Family decision-making dilemmas and optimized physician-patient communication pathways:Ethical conflicts and emotional burdens in surrogate decision-making,and communication needs within advance care planning frameworks;③ Impact of ICD-related cognitive biases on clinical decision-making.Conclusion The primary reasons for patients deciding to deactivate ICDs were painful shocks and caregiver burden.However,this decision was very emotionally distressing for families.It is critically necessary for healthcare providers to educate patients about ICDs and discuss end-of-life ICD deactivation.关键词
