胰岛功能评价在初发妊娠期糖尿病患者中的价值及影响因素研究
The value and influencing factors of islet function evaluation in primary gestational diabetes mellitus

Abstract
Objective To observe the islet function in patients with GDM,analyze the relation-ship between islet function indicators and blood glucose monitoring indicators,and analyze the influ-ence of relevant metabolic indicators on islet function,so as to provide evidence for better diagnosis and treatment of GDM.Methods Fifty pregnant women with GDM who received routine pregnancy examination in our hospital from April 2021 to April 2023 were selected as the study group,and healthy pregnant women who received routine pregnancy examination during the same period were se-lected as the control group.FPG,FINS,GSP,HbA1c,TG,TC,HDL,LDL,P,E2 and PRL were detected on an empty stomach during 24 to 28 weeks of pregnancy,and HOMA-IR and HOMA-βwere calculated.Results HOMA-IR,GSP and HbA1c in the study group were higher than those in the control group,with statistical significance(P＜0.05),while there was no statistical significance in HOMA-β between the two groups(P＞0.05).HOMA-IR was positively correlated with FPG,FINS and GSP,but not with HbA1c;HOMA was positively correlated with FINS,but not with FPG,GSP and HbA1c;In the study group,multiple linear regression analysis showed that P,E2,PRL,TG,TC and LDL levels were important factors affecting HOMA-IR.Conclusions The occurrence of GDM is closely related to insulin resistance in patients.The level of GSP can indicate the status of in-sulin resistance in patients.Hormones such as P,E2 and PRL will affect the degree of insulin resist-ance and lead to the occurrence of GDM,and patients with GDM are often accompanied by abnormal lipid metabolism.In the management of GDM patients,these factors should be considered compre-hensively and the islet function evaluation should be carried out in time to achieve individualized treat-ment and management.关键词
