Interpretation of Posaconazole Therapeutic Drug Monitoring Results and Evaluation of Effectiveness

Abstract
Objective To evaluate the effect of therapeutic drug monitoring(TDM)results interpretation in pharmaceutical practice,taking posaconazole as an example,and to promote the specialization and standardization of TDM results interpretation.Methods The data of posaconazole TDM in the First Affiliated Hospital of Soochow University from 2020 to 2023 were collected and analyzed,the results of TDM were interpreted,medication suggestions were provided for the factors affecting the concentration of posaconazole that were not up to the standard,and changes in the rate of posaconazole concentration reachingtarget were investigated.Results ① A total of 8 780 concentrations of posaconazole were detected in 1 655 patients,with concentrations ranging from 0.01 to 8.64 μg∙mL-1 and a median concentration of 0.9 μg∙mL-1(0.53 to 1.46),with an attainment rate of 75.24%,and an ineffective monitoring rate was 3.19%.② The main reasons for the non-attainment of posaconazole concentration were:detection time,dosage form,combination of drugs,gastrointestinal complications,hypoproteinemia,etc.;③ The interpretation of TDM results was carried out through standardization.The posaconazole concentration attainment rate was 83.34%by 2023,which was significantly higher than before(P<0.01),and the invalid monitoring rate was 2.1%,which was significantly lower than before(P<0.01).Conclusion The standardized implementation of TDM results interpretation can effectively improve the compliance rate of therapeutic drug concentration,guarantee drug efficacy.关键词
