Surgical treatment for anterior vaginal wall prolapse guided by integral theory
摘要
Abstract
Pelvic organ prolapse(POP)significantly affects women's quality of life.Anterior vaginal wall prolapse is the most common type,and its treatment faces many challenges.This article analyzes the surgical treatments for anterior vaginal wall prolapse based on the integral theory.The integral theory holds that the pelvic floor is an inter-related integrity,and anterior vaginal wall prolapse is closely related to the prolapse of other pelvic compartment,especially middle compartement.Clinically,anterior pelvic prolapse is often accompanied by stress urinary incontinence.There are various surgical procedures at present,each with its own advantages and disadvantages,such as traditional native tissue anterior midline colporrhaphy and paravaginal repair,whose recurrence rate is still the highest among all the pelvic compartments.Anterior pelvic reconstruction with mesh implantation can reduce the recurrence rate but has the risk of mesh-related complications.In clinical practice,it is necessary to comprehensively consider the patient's condition to select an appropriate surgical procedure.关键词
