老年弥漫大B细胞淋巴瘤治疗安全性的回顾性分析
A retrospective study on the safety of therapy in elderly patients with diffuse large B-cell lymphoma

Abstract
Objective To investigate the occurrence of adverse events and their influencing factors in elderly patients with diffuse large B-cell lymphoma after first-line treatment.Methods Elderly diffuse large B-cell lymphoma patients aged ≥65 years who received initial treatment in the Affiliated Cancer Hospital of Zhengzhou University from January 2017 to June 2023 were selected as the study subjects.Based on whether they experienced severe adverse events after first-line treatment,patients were divided into severe adverse event group(130 cases)and non-severe adverse event group(70 cases),and independent risk factors were identified through binary logistic regression analysis.Results The most common grade ≥3 hematologic adverse event after first-line treatment in elderly diffuse large B-cell lymphoma patients was a decrease in neutrophil count(32%),and the most common non-hematologic adverse event was infection(36%).Female gender(OR 6.618)and high levels of procalcitonin(OR 6.390)were identified as independent risk factors for the occurrence of severe adverse events after first-line treatment in elderly diffuse large B-cell lymphoma patients(P<0.05).The hemoglobin level in female patients(115.8±15.5)g/L was significantly lower than in male patients(124.7±21.4)g/L,the difference is statistically significant(P=0.001).Conclusion Risk factors for the occurrence of severe adverse events after first-line treatment in elderly diffuse large B-cell lymphoma patients include female gender,anemia,and high levels of procalcitonin.For elderly patients with advanced age and underlying heart disease,coronary artery CT angiography has guiding significance in the selection of treatment options.关键词
