不同临床风险评分对非瓣膜性房颤的识别价值比较
Comparison of the value of clinical risk scores in recognition of nonvalvular atrial fibrillation

Abstract
Objective To compare the value of CHADS2.CHA2DS2-VA,CHA2DS2-VASc and C2HEST scores in the recognitionof nonvalvular atrial fibrillation(AF)in people at high cardiovascular risk.Methods 54 934 hospitalized hypertensive patients were retrospectively enrolled.Calculate the CHADS2,CHA2DS2-VA,CHA2DS2-VASc and C2HEST scores of each patient according to medical records.The area under the receiver operating characteristic curve(AUC)was used to compare their AF recognition value in total population and different gender and age subgroups.And the Youden's index was applied to find the optimal cut-off value of the score.Results C2HEST showed better AF recognition value than other three scores,regardless of gender and age,whose AUC in the total population was 0.727(95％CI 0.723-0.731).The optimal cut-off value is 3 points.Conclusion C2HEST score has a better recognition value for AF than CHADS2,CHA2DS2-VA and CHA2DS2-VASc in hypertensive population.关键词
