Postoperative survival prediction model construction based on histopathological components for patients with early lung adenocarcinoma

Abstract
[Objective]To investigate the factors influencing survival after operation of early lung adenocarcinoma and to establish a prognostic model based on histopathological components,in order to provide more reference for subsequent individualized clinical diagnosis and treatment.[Methods]A total of 418 patients with stage I lung adenocarcinoma who underwent surgical treatment in Jiyuan People's Hospital from August 2018 to August 2020 were retrospectively included,and clinical and pathological characteristics were analyzed,as well as postoperative follow-up recurrence and survival.Univariate and multivariate analysis were used to analyze factors influencing postoperative recurrence free survival in early lung adenocarcinoma.A predictive model for postoperative recurrence free survival in early lung adenocarcinoma was constructed and its efficacy was analyed.[Results]Postoperative follow-up was 18 to 36 months,with a median follow-up time of 25.0 months.In this study,the recurrence-free survival rate of 418 patients was 80.62%(337/418)after 3-year follow-up.The results of univariate analysis showed that T stage,major histopathological component type,micropapillary component or not,and nodule CT feature type were related to the recurrence free survival of early lung adenocarcinoma after surgery(P<0.05).Logistic multivariate analysis showed that T stage,histopathological subtype,micropapillary component and CT characteristics of nodules were all independent influencing factors for postoperative relapse-free survival of early lung adenocarcinoma(P<0.05).Independent influencing factors of logistic regression model and P-value prediction probability were used to predict the recurrence free survival of early lung adenocarcinoma after surgery,and the areas under the curve were 0.654,0.717,0.601,0.632 and 0.866,respectively.[Conclusion]The postoperative survival of early lung adenocarcinoma is not only related to T stage and histopathological subtype,but also directly affected by pathological histological components.The data model constructed by using the above factors has shown good efficacy in predicting progression-free survival of patients,which is worthy of further analysis at work.关键词
