Abstract

Objective To investigate the early predictive effect of vancomycin nephrotoxicity biomarkers in vancomycin-associated acute kidney injury(VAKI).Methods A total of 40 rats were randomly divided into the blank control group(group A,without any drugs)and the vancomycin-induced groups at different times(groups B,C,D,E,F,G,and H,respectively,injected intraperitoneally with 400 mg/kg of vancomycin for 1,2,3,4,5,6,and 7 d,once a day),with five rats in each group.The serum and kidney tissue samples were taken 24 h after injection.Hematoxylin-eosin(HE)staining was used to observe the pathological changes of rats'renal tissue.Enzyme assay was used to detect the creatinine(SCr)levels in renal tissue and serum.Urease-glutamate dehydrogenase method was used to detect the urea nitrogen(BUN)levels in renal tissue and serum.Enzyme-linked immunosorbent assay(ELISA)was used to detect the levels of kidney injury molecule 1(KIM-1),neutrophil gelatinase-associated lipocalin(NGAL),interleukin-18(IL-18),and β2-microglobulin(β2-MG)in renal tissue and serum.Results Compared with those in group A,the renal interstitial edema worsened,cytoplasmic vacuolar changes,the cytoplasmic vacuolar changes in renal tubules increased,the number of apoptotic cells increased,and the degree of pathological damage to renal tissue worsened with the prolonged use of vancomycin in groups B-H;the expression levels of KIM-1,NGAL,IL-18,and β2-MG in serum and renal tissue significantly increased in groups B-H(P<0.05);the pathological scores significantly increased in groups C-H(P<0.05),and the expression levels of SCr and BUN in serum and renal tissue significantly increased in groups C-H(P<0.05).Conclusion The combined detection of biomarkers KIM-1,NGAL,IL-18,and β2-MG levels in serum and kidney tissue samples can predict VAKI early and accurately.
