Effect of Different Doses of Vitamin D on Insulin Resistance and Pregnancy Outcome in Gestational Diabetes Mellitus Patients:A Meta-Analysis

Abstract
Objective To investigate the effect of different doses of vitamin D on insulin resistance and pregnancy outcome in patients with gestational diabetes mellitus(GDM).Methods The randomized controlled trials(RCTs)related to the effects of different doses of vitamin D on insulin resistance and pregnancy outcomes in GDM patients in the PubMed,Embase,The Cochrane Library,CNKI,CBM,VIP,and WanFang databases were searched from the inception of the databases to April 2024.The quality of RCTs was evaluated by the Cochrane Handbook(Version 5.1.0),and Meta-analysis was conducted by the RevMan 5.3 software.Results A total of 14 RCTs were included,involving 1 657 patients,including 839 cases in the experimental group and 818 cases in the control group.Meta-analysis results showed that the reduction in homeostasis model assessment of insulin resistance[SMD=1.34,95%CI(1.10,1.58),P<0.000 01],reduction in fasting insulin[SMD=2.55,95%CI(2.06,3.03),P<0.000 01],and the cases with normal vitamin D levels[RR=0.30,95%CI(0.24,0.37),P<0.000 01]inthe experimental group were significantly higher than those in the control group.The incidence of adverse pregnancy outcomes[RR=1.98,95%CI(1.59,2.47),P<0.000 01]and the incidence of hypoglycemic reactions[RR=2.25,95%CI(1.42,3.55),P=0.000 5]in the experimental group were significantly lower than those in the control group.Conclusion Supplementing with high-dose vitamin D can significantly improve insulin resistance in GDM patients,correct vitamin D deficiency,and reduce the incidence of adverse pregnancy outcomes and maternal hypoglycemic reactions.关键词
