Abstract
Objective:To study the effect of laparoscopic anatomical hepatectomy(LAH)along the main hepatic vein in the treatment of patients with primary liver cancer.Method:The clinical data of 129 patients with primary liver cancer who admitted to Affiliated Hospital of Xuzhou Medical University from January 1,2014 to January 1,2024 were collected,64 patients treated with LAH through traditional approach from January 1,2014 to December 12,2018 were as the control group,and 65 patients treated with LAH along the main hepatic vein approach from January 1,2019 to January 1,2024 were as the observation group.The surgical indicators,liver function,immune function and complications were compared between two groups.Result:The operation time,hepatic portal occlusion time,drainage tube placement time and hospitalization time in the observation group were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group,the differences were statistically significant(P<0.05);at 7 d after operation,the alanine aminotransferase(ALT),aspartate aminotransferase(AST)and total bilirubin(TBIL)in two groups increased,but those in the observation group were lower than those in the control group,the albumin(ALB)in two groups decreased,but that in the observation group was higher than that in the control group,and the differences were statistically significant(P<0.05);at 7 d after operation,the immunoglobulin A(IgA)and immunoglobulin G(IgG)in two groups decreased,but those in the observation group were higher than those in the control group,the CD4+in two groups increased,and that in the observation group were higher than that in the control group,the CD8+in two groups decreased,and that in the observation group was lower than that in the control group,the differences were statistically significant(P<0.05);within 7 d after operation,the incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:LAH along the main hepatic vein approach in the treatment of primary liver cancer can improve surgical indicators,enhance liver function and immune function,and reduce the incidence of complications.关键词
