老年结直肠癌患者术后谵妄的影响因素
Influencing Factors of Postoperative Delirium in Elderly Patients with Colorectal Cancer

Abstract
Objective:To investigate the influencing factors of postoperative delirium(POD)in elderly patients with colorectal cancer.Method:A total of 115 elderly patients undergoing colorectal cancer surgery under general anesthesia admitted to Guangzhou First People's Hospital from March 2023 to March 2024 were selected as the study subjects.The patients'frailness before surgery was assessed by the Edmont frailness scale,and POD was evaluated by the unconsciousness assessment scale(CAM)after surgery.The patients were divided into delirium group(group D,n=21 cases)and non-delirium group(group N,n=94 cases)according to whether delirium occurred.After consulting the medical records of the two groups,univariate and multivariate logistic regression analysis was performed on the possible influencing factors of POD occurrence.Result:POD occurred in 21 cases(18.26%)of 115 elderly patients with colorectal cancer.Univariate results showed that there were statistically significant differences with preoperative frailness,age,co-morbidity,drug therapy,existing discomfort,cognitive function,anesthesia mode and anesthesia time between group D and group N(P<0.05).Multivariate logistic regression results showed that preoperative frailty,age,co-morbidity,drug therapy,existing discomfort,cognitive function,operation time and anesthesia time were the influencing factors for POD occurrence in elderly patients with colorectal cancer(P<0.05).Conclusion:POD accounts for a high proportion of elderly patients with colorectal cancer,and is affected by factors such as preoperative weakness,age,co-existing diseases,drug therapy,existing discomfort,operation time and anesthesia time,etc.Preventive measures should be developed according to the above controllable factors to reduce the incidence of POD.关键词
