原发性胆汁性胆管炎的中医证候研究
Analysis of TCM syndrome of primary biliary cholangitis

Abstract
Objective:To investigate the TCM syndrome differentiation patterns and the distribution characteristics of common syndromes in patients with primary biliary cholangitis(PBC),aiming to formulate effective symptom diagnostic guidelines for clinical practice.Methods:Through systematic epidemiological investigation,clinical data of 220 patients with primary biliary cholangitis(PBC)were collected.Cluster analysis was performed by utilizing the quick clustering method,followed by analysis of the distribution characteristics of TCM syndromes in PBC,and exploration of the distribution patterns of syndromes at different stages of the disease.Results:Firstly,the cluster analysis of 220 PBC patients identified six categories:Liver Qi Stagnation and Spleen Deficiency syndrome in 84 cases(38.18％),Liver and Kidney Yin Deficiency syndrome in 61 cases(27.72％),Blood Stasis and Meridian Obstruction syndrome in 36 cases(16.36％),Dampness-Heat Accumulation syndrome in 30 cases(13.63％),Spleen and Stomach Qi Deficiency syndrome in 6 cases(2.27％),and Qi Deficiency and Blood Stasis syndrome in 3 cases(1.36％).Secondly,the most common syndrome during the symptomatic phase was Liver Qi Stagnation and Spleen Deficiency,followed by Liver and Kidney Yin Deficiency.During the decompensation phase,Liver and Kidney Yin Deficiency syndrome was most common,followed by Blood Stasis and Meridian Obstruction.Liver and Kidney Yin Deficiency and Spleen and Stomach Qi Deficiency syndromes were observed at all stages of the disease.Thirdly,stage Ⅰ and Ⅱ were predominantly characterized by Liver Qi Stagnation and Spleen Deficiency Syndrome,stage Ⅲ by Liver and Kidney Yin Deficiency Syndrome,and stage Ⅳ by Blood Stasis and Meridian Obstruction syndrome.Spleen and Stomach Qi Deficiency syndrome could occur throughout the entire course of the disease.Conclusion:Common TCM syndromes in PBC patients include Liver Qi Stagnation and Spleen Deficiency,Liver and Kidney Yin Deficiency,Blood Stasis and Meridian Obstruction,and Dampness-Heat Accumulation.The occurrence of these syndromes is dependent on the different stages of disease progression.关键词
