Harmonizing and regulating Yin and Yang in the differentiation and treatment of elderly hypertension with insomnia based on kidney

Abstract
In the context of global population aging,clinical findings show that elderly hypertension is accompanied by more insomnia complications and deteriorating quality of life.The pathogenesis is still unclear,and the use of antihypertensive drugs combined with sedative hypnotic drugs has adverse drug reactions and dependence.Starting from the holistic concept of traditional Chinese medicine,Professor FENG Xiaojing puts forward the treatment view of"kidney-oriented and barmonizing and regulating Yin(阴)and Yang(阳)",and concludes that kidney-yin and kidney-yang deficiency are the core pathogenesis of hypertension and insomnia in the elderly,which are closely related to the liver,spleen and heart.The imbalance of Yin and Yang of the kidney then leads to the pathogenesis changes of Yin and Yin deficiency of the liver and kidney,spleen and kidney-yang deficiency,and heart-kidney disharmony.The treatment is mainly to correct the bias of kidney Yin and kidney Yang,taking into account the pathological products of liver Yang,heart fire,phlegm dampness and so on.Professor FENG studies ancient books deeply and is not limited to them.She creates a prescription flexibly:for the pathogenesis of kidney Yin deficiency and liver Yang upward disturbance,nourishing the kidney Yin and flattening the liver and suppressing Yang are the therapeutic principles and Modified Jianling decoction is prescribed;For the pathogenesis of fire failure of Mingmen and loss of spleen Yang,warming and tonifying kidney Yang,strengthening spleen and eliminating phlegm,calming and suppressing the floating Yang are the therapeutic principles and Self-made Bushen Dingxuan decoction is prescribed;for the pathogenesis of kidney Yin deficiency,heart Yin deficiency and heart Yang hyperactivity,nourishing kidney Yin,clearing heart fire are the therapeutic principles and the method of communicating heart and kidney is applied,and Modified Huanglian Ejiao decoction is prescribed.In his clinical diagnosis,FENG pays attention to the combination of traditional Chinese and Western medicine,examines and seeks the cause of the syndrome,and treats the senile simple systolic hypertension syndrome of kidney-yang deficiency with warming Yang and tonifying kidney;In the treatment of the elderly hypertensive myocardial hypertrophy,she is good at eliminating accumulation by removing blood stasis and expelling phlegm and paying attention to the differentiation of zang-fu syndrome and liver-yang hyperactivity to clear liver-fire,soothe liver-depression,and those with strong heart fire are treated with clearing heart-fire and nourishing heart-yin;the method of regulating qi and regulating emotions is given attention to achieve calming the mind and emotion;In the elderly patients with increased diastolic blood pressure,medicinals for relieving water and dampness are supplemented to reduce blood volume and relieve cardiac load,the patients with phlegm and blood stasis interformation can be given treatment for eliminating phlegm and promoting blood circulation and multiple methods can be used to reduce blood pressure and relieve accompanying symptoms.关键词
