Abstract
Diarrheal irritable bowel syndrome(IBS-D)is a common functional gastrointestinal disease characterized by recurrent abdominal pain,accompanied by abnormal defecation or changes in defecation habits.At present,Western medicine commonly uses spasmolytics,antidiarrheal agents,visceral analgesics,antidepressants and other drugs to treat this disease,but the clinical effect is not good.The pathogenesis of this disease is complicated,and Western medicine believes that its occurrence is related to immune imbalance,brain-intestinal axis disorder,intestinal flora disorder and social psychological factors.Among them,there is a close relationship between intestinal flora and immune imbalance.Studies have shown that changes in intestinal flora and metabolites can affect the body's immune system,and then affect the prognosis of IBS-D patients.The theory of"the spleen is the guard"in traditional Chinese medicine shows that the spleen has the role of resisting external pathogens and protecting the body,which is consistent with the function of the immune system in modern medicine.The physiological function of the spleen,which is responsible for transporting and absorbing water and grain essence to disperse the whole body,is consistent with the function of intestinal flora to decompose and absorb food.Therefore,in this paper,the pathogenesis of IBS-D is discussed from the perspective of"intestinal microecology-immunoregulation",and it is believed that intestinal microecology will affect the body's immune regulatory function,thus participating in the occurrence of IBS-D.Combining this mechanism with the theory of"the spleen is the guard"in traditional Chinese medicine,the treatment of IBS-D starts from strengthening spleen and invigorating Qi(气),and clearing heat and dispelling dampness,which provides a new entry point for the targeted treatment of this disease in traditional Chinese medicine.关键词
