Exploring the pathogenesis and treatment of Bentun disease based on the theory of"one Qi circulation"

Abstract
Bentun disease(奔豚)refers to the disease where one feels that there is an upward flow of air from the lower abdomen to the throat and pharynx.This paper explores the pathogenesis and treatment of Bentun disease based on HUANG Yuanyu's"one Qi(气)circulation"theory,and believes that the main pathogenesis of Bentun disease is the reversal of Qi flow due to Yang(阳)deficiency,wetness in the soil,rising liver fire,water vapor rising,wood being repressed by cold,and cold and damp not being subdued.The Yang deficiency and wetness in the soil cause the two fires not to descend,leading to the rising of liver fire.The Yang Qi becomes deficient and fails to transform into Qi,causing cold water to accumulate in the kidneys,leading to the rising of the dynamic Qi in the kidney interstitial space.With prolonged Yang deficiency,cold and damp gather,forming a kidney-related Bentun disease.The Yang deficiency and cold and damp cause the wood to become stagnant,leading to the rising of wind.The rising of wind leads to Bentun Qi.It can be seen that the failure of the earth to coordinate the other four internal organs leads to reversal of the ascending and descending process,causing stagnation of the"one Qi circulation",and thus the symptoms of Bentun Qi rising.The treatment of Bentun disease should follow the principle of treating the root of the disease.The goal is to warm the Yang Qi and strengthen the middle Qi,restore the middle Qi's ability to rotate and regulate the ascending and descending process as the core,and use different methods of treatment based on the different deviations of the other four internal organs of coordination.When the phlegm fire rises upwards,it should be subdued with the Bentun decoction(奔豚汤)to lower the rebellious phlegm fire,when the water Qi ascends,it should be regulated with the Linggui Zaogan decoction(苓桂枣甘汤)to nourish the soil and drain water;when the wood Qi rushes upwards,it should be regulated with the Guizhi Jiagui decoction(桂枝加桂汤)to warm the Yang and transform the Qi to calm the surging wood Qi;when the cold accumulation does not subdue,it should be regulated with Longzhu plaster(龙 珠 膏)to warm the spleen and dry the soil,and transform the accumulation and calm it down.Therefore,when the middle Qi is strong and well-functioning,the axis rotates smoothly,the ascending and descending movements are harmonious,and the Qi circulates smoothly like a ring,and the rebellious Qi will naturally dissipate.The article outlines the theoretical concept of the theory of"one Qi circulation",explores the core pathogenesis and disease characteristics of Bentun based on this theory,and discusses the treatment principles and methods for Bentun disease based on the theory of"one Qi circulation",providing theoretical references for clinical treatment.关键词
