以周围神经损害为首发症状的结节病一例并文献复习
A case of sarcoidosis presenting with peripheral nerve damage as the initial symptom and literature review
摘要
Abstract
Objective To analyze the clinical characteristics,diagnosis,and treatment of neurosarcoidosis.Methods A retrospective review was conducted on a rare case of peripheral nerve damage due to sarcoidosis diagnosed and treated in our hospital.Results The patient presented with electric-shock-like pain in the lower back and dorsal region and peripheral facial nerve paralysis.Routine chest CT examination revealed multiple lesions in both lungs and enlargement of hilar and mediastinal lymph nodes.Lung pathological samples were taken,and non-caseating granulomatous changes were found.After excluding tuberculosis,tumors,and other diseases causing granulomatous changes,we considered it a case of sarcoidosis involving the nervous system(neurosarcoidosis).The patient im-proved after glucocorticoid treatment and has been in normal condition since then.Conclusion Sarcoidosis is a relatively rare multisystem granulomatous disease that mostly affects the lungs and intrathoracic lymph nodes.In-volvement of nervous system is rarely reported.In this case,the patient had peripheral nerve damage as the initial symptom,which may result in misdiagnosis.关键词
