摘要

Abstract

Objective In this study,data-independent acquisition(DIA)and bioinformatics analysis were performed to reveal the potential mechanisms of acupuncture in relieving cancer-related pain(CRP).Methods A randomized controlled trial was conducted in 6 hospitals in Guangdong province.Cancer patients with moderate to severe pain were randomly divided into the acupuncture group and the sham acupuncture group,with 9 cases in each group.Both the acupuncture group and the sham acupuncture group were treated with oral oxycodone hydrochloride sustained-release tablets for analgesi,and received 7 times of acupuncture/sham acupuncture within 3 weeks,2 to 3 times a week,with a follow-up period of 4 weeks.The patients' pain was recorded with the numeric rating scale(NRS),including the most severe pain score,mean pain score,current pain score,and total dose of opioid use.Peripheral blood samples before and after treatment of two groups of patients were collected for further serum proteomics testing.Differential expression analysis was used to identify key differential expression proteins(DEPs).Gene set enrichment analysis(GSEA),gene ontology(GO),and Kyoto encyclopedia of genes and genomes(KEGG)functional enrichment analysis were used to identify the biological pathways that might be regulated by acupuncture.The module genes most closely related to the most severe pain score,mean pain score,current pain score,and the total dose of opioid use were identified by weighted gene co-expression network analysis(WGCNA).The core targets were determined by protein-protein interaction(PPI)network.Finally,the trend analysis of protein expression was used to clarify the differences and potential mechanisms between acupuncture and sham acupuncture in regulating CRP.Results The acupuncture group significantly reduced the most severe pain scores compared with the sham acupuncture group in cancer patients(P<0.05).A total of 36 peripheral blood samples were obtained before and after treatment in the acupuncture and sham acupuncture groups.DEPs analysis showed that 84 proteins were up-regulated and 10 proteins were down-regulated.GSEA enrichment analysis showed that acupuncture significantly down-regulated chemokine secretion,tumor necrosis factor(TNF)production,and acute inflammatory response pathways.KEGG enrichment analysis of 84 up-regulated proteins showed that acupuncture was able to up-regulate immune response and immune effector process,and KEGG enrichment analysis of 10 down-regulated proteins showed that acupuncture was able to down-regulate the activity of complement component C1q combined with hydrolase and complement binding.WGCNA analysis identified the blue module as the key module,whose genes were significantly positively correlated with the most severe pain scores,and GO functional enrichment analysis showed that these genes were enriched in C1q binding hydrolase activity and complement binding signaling pathways.Trend analysis of protein expression showed that the Cluster 5 gene cluster was balanced at baseline between the acupuncture group and the sham acupuncture group,significantly up-regulated after sham acupuncture,and was at the level between baseline and after sham acupuncture intervention.Further analysis revealed that these genes were closely related to the Ras-related protein 1(RAP1)signaling pathway.Conclusion Acupuncture can reduce the most severe pain score in cancer patients.Acupuncture is able to reduce the level of inflammation and enhance immunity in CRP patients,which may be closely related to signaling pathways such as RAP1.
