术前CT评估肿瘤生长模式对原发性肝癌复发转移的预测价值
The Predictive Value of Preoperative CT Evaluation of Tumor Growth Pattern for Recurrence and Metastasis of Primary Liver Cancer

Abstract
Objective:To explore the predictive value of preoperative CT evaluation of tumor growth pattern in recurrence and metastasis of primary liver cancer.Methods:A total of 107 patients with non-metastatic primary liver cancer diagnosed and treated in our hospital from January 2021 to January 2023 were selected.All patients underwent preoperative CT examination and radical resection,followed by postoperative follow-up.They were divided into an occurrence group(33 cases)and a non-occurrence group(74 cases)according to whether recurrence and metastasis occurred within 1 year after surgery.Clinical data and preoperative CT evaluation of tumor growth pattern were compared between the two groups,and factors affecting recurrence and metastasis of primary liver cancer and the predictive value of preoperative CT assessment were analyzed.Results:The proportions of pathological TNM stage Ⅲ,preoperative CT assessment of tumor growth pattern as G2 type,intratumoral necrosis,pathological growth pattern as massive type,and incomplete/no capsule were higher in the occurrence group than in the non-occurrence group(P＜0.05).Multivariate Logistic regression analysis showed that these factors were risk factors for recurrence and metastasis of primary liver cancer(P＜0.05).The AUC of preoperative CT assessment of tumor growth pattern in predicting recurrence and metastasis of primary liver cancer was 0.924,with a sensitivity of 90.91％and a specificity of 97.30％.Conclusion:Preoperative CT assessment of tumor growth pattern has a high predictive value for recurrence and metastasis of primary liver cancer and is significantly related to the growth pattern of massive pathological tumors with poor prognosis,providing an important basis for clinical surgical decision-making and postoperative management.关键词
