Evaluation of Anterior Cruciate Ligament Injury in Patients with Tibial Plateau Fractures Based on Tibial Plateau Widening Distance and Joint Effusion Volume Measured by MRI

Abstract
Objective:To investigate the value of tibial plateau widening distance(TPW)and joint effusion volume measured by magnetic resonance imaging(MRI)in the diagnosis and prediction of anterior cruciate ligament(ACL)injury in patients with tibial plateau fractures.Methods:This study included 107 patients with tibial plateau fractures from January 2023 to June 2024.According to whether they were accompanied by ACL injury,they were divided into injury group(n=68)and non-injury group(n=39).TPW and joint effusion volume were measured by MRI.The MRI features and general data were compared between groups.Logistic regression analysis was used to analyze the influencing factors of ACL injury in patients with tibial plateau fractures.Receiver operating characteristic(ROC)curve was adopted to analyze the evaluated efficiency of TPW and joint effusion volume on ACL injury in patients with tibial plateau fractures.Spearman rank correlation analysis was performed to analyze the correlation between TPW,joint effusion volume and ACL injury degree.Results:Logistic regression analysis showed that TPW and joint effusion volume were independent risk factors for ACL injury in patients with tibial plateau fractures(P<0.05).ROC curve analysis indicated that TPW,joint effusion volume and combined detection had certain efficiency on evaluating ACL injury in patients with tibial plateau fractures(P<0.05),and the areas under the curves(AUCs)were 0.962,0.815 and 0.985 respectively.The predictive efficiency of combined diagnosis was the highest(sensitivity=98.53%,specificity=89.74%).Among the 68 patients with ACL injury,there were 33 cases(48.53%)of grade 1,17 cases(25.00%)of grade 2 and 18 cases(26.47%)of grade 3.Spearman correlation analysis found that TPW and joint effusion volume were positively correlated with ACL injury degree(P<0.05).Conclusion:MRI can measure the enhancement of TPW and the increase of joint effusion in patients with tibial plateau fractures complicated with ACL injury,and both indicators are independent risk factors for ACL injury in patients with tibial plateau fractures.Combined measurement has high efficiency on evaluating ACL injury.In addition,TPW and joint effusion volume are also correlated with the degree of ACL injury.关键词
