超声造影到达时间参数成像对乳腺结节良恶性的诊断效能
Diagnostic Efficacy of Contrast-enhanced Ultrasound Arrival Time Parameter Imaging in Benign and Malignant Breast Nodules

Abstract
Objective:To explore diagnosting contrast-enhanced ultrasound arrival time parameter imaging(ATPI)in benign and malignant breast nodules.Methods:The results of conventional ultrasound and contrast-enhanced ultrasound were retrospectively analyzed in 110 cases(114 nodules)of breast nodules,which were divided into benign group and malignant group according to pathological diagnosis.The ATPI characteristics,parameters and time-intensity curve(TIC)parameters of the two groups were compared.The diagnostic efficiency was evaluated by ROC curve and Logistic regression analysis,and the qualitative diagnostic model of breast nodules was constructed.Results:There were significant differences between the malignant group and the benign group in perfusion time,perfusion order,lesion size,perfusion defect,nourishing vessels and ATPI parameter ΔT(P＜0.05).The peak time and initial increment time of TIC parameters in the malignant group were shorter than those in the benign group,and the area under the curve and gradient were higher than those in the benign group(P＜0.05).The AUC of ATPI parameter ΔT in the diagnosis of malignant breast nodular was 0.872,which was higher than TIC parameter.Logistic regression analysis showed that perfusion time(fast forward),lesion enlargement after perfusion,perfusion defect and nourishing vessels were the risk factors for malignant breast nodules,and ΔT was the protective factor(P＜0.05).The constructed Logistic regression model was well fitted in Hosmer-Lemeshow test,with ROC curve specificity of 0.870,sensitivity of 0.956 and AUC of 0.965(95％CI:0.938～0.992).Conclusion:Logistic regression model based on ATPI characteristics can effectively diagnose benign and malignant breast nodules and improve the accuracy of qualitative diagnosis of breast nodules.关键词
